What We Learned Deploying AI in Clinical Environments

Real-world clinical settings demand a different standard of reliability, compliance, and usability than typical SaaS. Here's what we learned.

This is not a marketing workflow

Deploying AI in a healthcare clinic is nothing like deploying it in a marketing department or a customer support queue. In those environments, if the AI gets something wrong, someone fixes a typo or re-sends an email. The cost of failure is low and the tolerance for imperfection is high.

In a clinic, a provider sees 25 to 30 patients a day. Each encounter generates a clinical note that becomes part of the medical record, drives billing, and may be reviewed by other physicians, insurance companies, or — in worst-case scenarios — attorneys. The margin for error is not generous. The tolerance for downtime is zero.

We've deployed AI documentation tools in allergy clinics, dermatology practices, primary care offices, and multi-specialty groups. Every deployment taught us something. Here's what we know now that we didn't know at the start.

Reliability beats intelligence, every time

The first instinct when building AI for healthcare is to make it as smart as possible. More nuanced clinical reasoning. More sophisticated language understanding. More impressive output.

That instinct is wrong.

What providers actually need is a tool that works correctly 99 percent of the time. Not a tool that produces brilliant output 90 percent of the time and confusing output the other 10 percent. A physician who can't trust the tool will stop using it — usually within the first three days.

We learned this the hard way. Early versions of our system tried to be too clever with clinical assessments. The output was impressive when it worked, but when it missed — when it inferred a diagnosis the provider hadn't stated, or structured a plan in an unexpected way — it eroded trust immediately. One bad note in a day of 30 patients is enough for a provider to go back to typing manually.

The fix was counterintuitive: we made the AI less ambitious. We focused on accurately capturing what was said rather than inferring what was meant. Reliability went up. Usage went up. Trust went up. And over time, with that foundation of trust, we were able to add intelligence back in — carefully, incrementally, always with the provider in control.

Speed is a clinical requirement

In a SaaS product, a 10-second loading time is annoying. In a clinic, it's disqualifying.

A provider finishes a patient encounter and has roughly 60 to 90 seconds before the next patient is ready. In that window, they need to review the note, make any edits, and move on. If the AI takes 10 seconds to generate the note, the provider is already behind. If it takes 30 seconds, the provider has mentally moved on and will "catch up on notes later" — which means the tool has failed.

We target sub-5-second note generation. That sounds like a small number, but achieving it consistently across varying encounter lengths, different specialties, and variable network conditions required significant engineering investment. We had to optimize the entire pipeline — audio processing, transcription, clinical structuring, and rendering — to hit that target reliably.

Seconds add up. Over 30 patients, a 10-second delay per note is 5 minutes. A 30-second delay is 15 minutes. And 15 extra minutes of waiting in a physician's day doesn't register as "15 minutes." It registers as "this tool is slow" and then "this tool is gone."

Integration is everything

The best AI note in the world is useless if the provider has to copy and paste it into their EHR. It sounds trivial. It's not.

Providers live inside their electronic health record — Epic, Cerner, athenahealth, eClinicalWorks, and dozens of others. Their entire workflow revolves around it. Any tool that exists outside the EHR is an extra tab, an extra step, an extra thing to remember. And in a 15-minute patient encounter, "extra" means "ignored."

We built integrations that push the generated note directly into the correct fields of the EHR. Not as a blob of text — as structured data mapped to the right sections. HPI goes in HPI. ROS goes in ROS. Assessment and plan go where assessment and plan belong. The provider opens their EHR after an encounter and the note is already there, in the right place, ready for review.

Building these integrations is tedious, unglamorous work. Each EHR has its own API quirks, its own certification requirements, its own deployment model. But it's the difference between a tool that providers actually use and a tool that gets uninstalled after the trial period.

Workflow fit over feature count

Early on, we made the classic product mistake: we shipped features. Lots of them. Customizable note templates. Multiple output formats. Configurable section ordering. Specialty-specific vocabulary settings. We thought more options meant more value.

Providers didn't want options. They wanted fewer decisions, not more. They wanted the tool to work the way they already worked — and to require as close to zero configuration as possible.

We stripped the product down. Three features, done perfectly, beat fifty features done adequately. Listen to the encounter. Generate the note. Put it in the EHR. That's it. Every feature we've added since has to pass one test: does this make the provider's day shorter, or does it just make our feature list longer?

The providers who use our tool most consistently are the ones who forget it's there. That's the highest compliment in clinical software — invisibility.

Compliance is not a feature — it's a foundation

In typical SaaS, compliance is something you add before a big enterprise deal. In healthcare, it's something you build on day one or you rebuild everything later.

HIPAA compliance isn't a checkbox. It's a set of constraints that affects every architectural decision — where data is stored, how it moves, who can access it, how long it's retained, how it's encrypted at rest and in transit, and what happens when something goes wrong.

We built with compliance as a structural requirement, not an afterthought:

  • BAAs (Business Associate Agreements) signed with every infrastructure provider and subprocessor before a single byte of patient data touches the system.
  • End-to-end encryption — data encrypted in transit (TLS 1.2+) and at rest (AES-256). Audio is processed and discarded. Nothing persists that doesn't need to.
  • Audit logging for every access, every modification, every export. If a compliance officer asks "who accessed this record and when," we can answer in seconds.
  • Role-based access control so providers see their patients and only their patients. Practice administrators see aggregate data, not individual records.
  • Incident response procedures documented, tested, and ready before we onboarded our first provider.

This work isn't exciting. It doesn't demo well. But it's the reason healthcare organizations trust us with their data — and the reason we can sign contracts with compliance-conscious health systems without scrambling to retrofit security controls.

Training and trust

Physicians are not early adopters by nature — and for good reason. When your job involves making decisions that affect human health, caution with new tools is a feature, not a bug.

We learned that onboarding has to accomplish three things, and it has to do all three in under an hour:

First, prove it works. Not with a slide deck. With their patients, in their exam room, on their first day. If the first five encounters produce accurate notes, the provider will give it a real chance. If the first note is wrong, you've lost them — possibly permanently.

Second, keep the provider in control. Every note is a draft until the provider approves it. They can edit anything. They can reject the entire note and write their own. The AI is an assistant, not an authority. This isn't just a UX choice — it's a clinical and legal requirement. The provider is the author of record, always.

Third, make the training disappear. If onboarding takes a full day, you've failed. Providers don't have a full day. They have the 30 minutes between their morning huddle and their first patient. The tool has to be intuitive enough that those 30 minutes are sufficient — and that by encounter five, the provider doesn't need to think about the tool at all.

Advice for other teams building AI for healthcare

If you're building or deploying AI in clinical environments, here's what we'd tell you over coffee:

Be boring. Healthcare doesn't need innovation theater. It needs tools that work the same way every time, in every encounter, without surprises. Boring is a compliment.

Be reliable. Uptime isn't a nice-to-have. If your system goes down during a clinic session, providers have to stop seeing patients or revert to manual documentation. Either outcome destroys trust. Build for five-nines availability and mean it.

Be fast. Clinical workflows are measured in seconds, not minutes. Optimize aggressively. Benchmark obsessively. If a provider ever has to wait for your tool, you've already lost.

Don't impress — simplify. Nobody in a clinic cares about your model architecture or your training data pipeline. They care about one thing: does this make my day shorter? If the answer is yes, they'll use it. If the answer is "yes, but you also have to learn these 12 features," they won't.

The best AI in healthcare is the AI that no one notices. It does its job, the provider does theirs, and the patient gets better care because neither of them is fighting with software.
